Just a sort of bare bones timeline so that I don't forget anything like a champ.
Joanne's gender is the only (initial) difference in this universe, so when Dio was adopted by the Joestars he kind of won simply by existing. Joanne couldn't inherit any of her family's property on her own, and so long as she never married it was all there for Dio to metaphorically roll around in.
She was no threat at all to him - in fact, she was a nothing, and for most of their lives that's exactly what Dio regarded her as. Their relationship was cordial but distant; on occasion he indulged in bullying her a bit, but never anything extreme enough that it couldn't be written off as the usual sibling squabbles. Because Dio had never really displayed any serious ill will towards her, Joanne never considered the way her suitors all seemed to quickly disappear anything but poor luck.
Things changed in Dio's final year of college. While he was content to continue getting what was essentially a free ride under George, he'd come to realize that even if Joanne never married, he would be responsible for taking care of her once George had passed on. Rather than waste any of his inheritance on his sister, he began to poison her.
Joanne quickly became an invalid, confined to her room and often unable to even leave her bed. During this time she began to research the stone mask which had belonged to her mother, as a way to alleviate her boredom. She discovered Dario Brando's letter in one of her research books and gradually began to wonder if both he and herself were a victim of Dio's machinations.
What followed was several months of isolation and gaslighting as Dio did his damnedest to convince his sister that her chronic illness was slowly causing her to lose her mind. It would have actually worked if Dio hadn't forgotten how much of a tomboy she'd been in her youth - rather than accept reality as it seemed to be, Joanne climbed out of her window and ran off to London to find definitive proof that she was being poisoned...and also an antidote.
Thankfully, unlike her male counterpart, Joanne had the foresight to bring a fucking gun to Ogre Street. While there she encountered Speedwagon who, impressed by a noblewoman so determined to find the truth that she'd stalk through London's roughest neighborhood shooting thugs in the kneecap, agreed to show her to Wang Chen's shop and continue on as her protector because you can really only jam so many bullets into a purse.
Meanwhile, Dio had been tearing up both the Joestar manor and the general vicinity in a desperate search for Joanne before she could do any real damage to him. During this search he discovered her notes on the stone mask...which means that shit pops off super hard when she comes home to confront him.

Jamming her physical description here as well because why not. Unfortunately for Joanne, she's just as much of a physical standout as her male counterpart; she's roughly six feet tall and has the sort of awkward athletic build that makes her as far from the ideal Victorian lady as possible when it comes to appearance. Her life is hard.
Also regardless of what some of her icons say to the contrary SHE HAS SHORT HAIR. It had to be cut after the injuries she incurred when the Joestar manor burnt down, and really doesn't do much for her femininity because it was a bunch of doctors clipping it. Alas.
